You'll simply fill in both parts, almost ALL doctor's forms I've ever filled out have 2 spots to list 2 healthcare providers.

Thanks for the responses guyes, but not really the input I was hoping for. I'm not into renting a house and don't care about riding a bus ever, I'm always happy to pay the $12 to park and take my own car. We're not really going for Disney world, did that last year, it just looks like a cool place and my kids would love the decor and activities. And of course I never leash my own dog at home either, she knows better, but in a strange place with 100 other barking dogs, it is totally unacceptable not have your pet there not on a leash or in a carrier, where they are safe, other pets are safe from them and other people.

You folks have lost it! I just woke up and 2 of the 3 kids did as well. Man 6 a.m. is gonna' be hard in the morning. We gotta get out of the house today, been here too dand long! I'm driving all the way to Cobb for come lunch time fun... I just hope their school is in today.

I leave p.com for 2 hours to go watch a movie with the husband that I've been dying to see and I come back to this...did I get ripped off or what? On second though, the daily drop off rate is $20 these days and I have plenty of empty crates around here, I could totally handle a few dozen little rugrats!
