It is the law to fill the form out. If you do not they will come to your door. Mine got coffee spilled all over it. I called to request another one. They said they would send it out and I never got it. I did get the guy though. I answered all of his simple questions. I The only part I froze on was the birthdays. I messed them up like five times. lol
