Paulding- physical form? for football

My cousin is looking fpr a form for a physical. The coach said it was on the BOE site, but we can not find it anywhere.

Any help would be great

Looks like the right one to me. Might go to one of the high school websites and access their athletic info. You should find one there and you can compare to make sure this is the correct one.

All schools use the same form, middle or high. Just not likely to find it on a middle school site since they no longer officially have sports.

However it should be on the Jr. Spartans web page.

There is still middle school sports. They are just not run through the middle schools anymore. The Jr. programs are great, and it is fantastic that the high schools stepped up and took over.

I wish folks would stop saying "there isn't any middle school sports anymore." There are, the programs are just not run by the middle schools.

Check with Coach Glanton, Head Coach at South. I know they (South) had the forms a few weeks ago...
